Coverage expansion for GLP-1 drugs

The policy targets a class of medications known as GLP-1 drugs, which include widely used treatments like Ozempic, Wegovy, and Zepbound. However, Ozempic is omitted from the list of eligible GLP-1 medications.

These drugs were originally developed to help manage blood sugar in people with Type 2 diabetes, but they have also gained widespread attention for their ability to support weight loss. The cost difference is what's drawing the most attention to the expanded coverage. Medicare enrollees who have Part D coverage and a BMI of 27 or higher plus a qualifying health condition are eligible for GLP-1 medications at $50/month.

For seniors currently paying out of pocket, that kind of reduction translates to thousands of dollars in annual savings. Even for those with partial coverage, lower negotiated pricing should ease monthly expenses and make long-term treatment more manageable.

Growing demand for GLP-1 medications

Interest in GLP-1 medications has exploded across the U.S. A 2025 survey found that roughly 1 in 8 American adults reported using a GLP-1 drug, with strong demand among older Americans.

Limits of coverage

Despite the headline savings, not every Medicare beneficiary may see the full benefit. Coverage depends on specific plan structures, including whether a drug is included on a plan's formulary and what conditions must be met for approval. Specifically, enrollees must have Part D and a BMI of 27 or higher, along with a qualifying medical condition.

The covered medications are also limited. The $50 copay applies to Wegovy (oral and injectable), the Zepbound KwikPen, and the Foundayo pill, but not Ozempic, a widely used GLP-1.

Temporary access

Eligible Medicare enrollees should note that reduced copay costs are temporary. The Medicare-GLP-1 Bridge, as it's known, is only available from July 1, 2026, through December 31, 2027.

Expanding coverage for expensive medications raises questions about long-term costs. While lower prices benefit individuals, the overall financial impact on Medicare depends on how widely the drugs are used and how pricing agreements are structured.

Increased utilization could drive higher total spending, even as per-patient costs decline. Balancing affordability with sustainability will likely remain part of the conversation.
