Beginning next year, Medicare beneficiaries will see prices fall for 15 key medications. Late in 2025, the U.S. Centers for Medicare & Medicaid Services (CMS) announced the list of drugs that would receive negotiated price cuts kicking in on Jan. 1, 2027.
The cuts will save Medicare between $8.5 billion and $12 billion annually and provide $685 million in out-of-pocket relief to millions of beneficiaries. Find out which drugs made the cut and whether the price reductions will help you keep more of your money.

Ozempic; Rybelsus; Wegovy
- 2027 price: $274
- Percentage reduction: 71%
These drugs have gained fame for their role in managing overweight and obesity. However, they also are used to treat Type 2 diabetes and cardiovascular disease. The 2024 list price was $959. In 2027, that will drop to $274 for a 30-day supply of the prescription.
Trelegy Ellipta
- 2027 price: $175
- Percentage reduction: 73%
Trelegy Ellipta is used to treat both asthma and chronic obstructive pulmonary disease. The 2024 list price was $654.
Xtandi
- 2027 price: $7,004
- Percentage reduction: 48%
Xtandi is used to treat prostate cancer and will cost consumers $7,004 in 2027. The 2024 list price was a shocking $13,480.

Pomalyst will see a 60% price reduction, down to $8,650, compared to $21,744 in 2024. It is used to treat Kaposi sarcoma and multiple myeloma, both of which are types of cancer.
Ofev
- 2027 price: $6,350
- Percentage reduction: 50%
A treatment for idiopathic pulmonary fibrosis, Ofev carried a 2024 list price of $12,622 — more than twice what patients will pay in 2027.
Ibrance
- 2027 price: $7,871
- Percentage reduction: 50%
At $15,741 in 2024, Ibrance — a breast cancer treatment — will see its price cut in half under the new negotiated rate.
Linzess
- 2027 price: $136
- Percentage reduction: 75%
The 2024 list price for Linzess was $539. The drug, which treats chronic idiopathic constipation and irritable bowel syndrome with constipation, will drop by 75% in 2027.
Calquence
- 2027 price: $8,600
- Percentage reduction: 40%
Calquence treats two blood cancers: chronic lymphocytic leukemia/small lymphocytic lymphoma and mantle cell lymphoma. Its 2024 list price of $14,228 will fall 40% by 2027.
Austedo; Austedo XR
- 2027 price: $4,093
- Percentage reduction: 38%
Used for chorea in Huntington's disease and tardive dyskinesia, these drugs had a 2024 list price of $6,623 before the 38% reduction takes effect.

- Percentage reduction: 83%
Few drugs on this list saw steeper cuts. Breo Ellipta, which treats asthma and chronic obstructive pulmonary disease, will drop from a 2024 list price of $397 to just $67.
Xifaxan
- 2027 price: $1,000
- Percentage reduction: 63%
Prescribed for both hepatic encephalopathy — a brain dysfunction tied to liver disease — and irritable bowel syndrome with diarrhea, Xifaxan's 2024 price of $2,696 will fall by nearly two-thirds.
Vraylar
- 2027 price: $770
- Percentage reduction: 44%
The 2024 list price for Vraylar was $1,376. The medication, used to manage bipolar 1 disorder, major depressive disorder, and schizophrenia, will be nearly halved by 2027.
Tradjenta
- 2027 price: $78
- Percentage reduction: 84%
At $488 in 2024, Tradjenta was already a relatively affordable Type 2 diabetes treatment. Its 84% price cut brings it down to $78, one of the sharpest reductions on the list.
Janumet; Janumet XR
- 2027 price: $80
- Percentage reduction: 85%
These Type 2 diabetes drugs had a combined 2024 list price of $526. The 85% reduction (the largest of any medication negotiated) brings them to $80 in 2027.
Otezla; Otezla XR
- 2027 price: $1,650
- Percentage reduction: 65%
Treating oral ulcers in Behçet's disease, plaque psoriasis and psoriatic arthritis, these drugs will drop from $4,722 in 2024 to $1,650. That's a 65% reduction.
Another way Medicare drugs are becoming cheaper
Medicare drugs also have become cheaper for some people thanks to a separate change that went into effect this year.
Beginning in 2026, there is a $2,100 cap on out-of-pocket prescription costs on covered prescription drugs for everyone with Medicare Part D coverage. This replaces the old untapped catastrophic coinsurance of 5%, which ran through the end of the calendar year.
The new cap means Medicare beneficiaries with very expensive prescription drugs are likely to save a lot of money compared to previous years.

It's important to note that the prices on this list are the maximum fair prices manufacturers must charge Medicare itself. That means the cost you pay will depend on your specific plan.
