Your Medicare Advantage Flex Card Will Start Getting Declined on These Items in 2027

New flex card rules could trigger more checkout declines

A Medicare Advantage flex card may look like a prepaid spending card, but it is not a government-issued Medicare benefit or free cash. It is a tool some private Medicare Advantage plans use to deliver specific supplemental benefits, such as allowances for approved over-the-counter products, groceries, or utilities.

Starting with 2027 coverage, tighter point-of-sale controls could make the card less flexible than some members expect. For someone living on just Social Security, an unexpected decline at checkout could create a real budgeting problem. Here is what could be rejected and how to prepare.

Purchases outside of your plan's covered categories

Under the 2027 Medicare Advantage final rule, cards must be electronically linked to plan-covered items and services. A real-time identification system must verify coverage at the point of sale.

That means having money left on the card may not be enough. If the purchase falls outside the benefits included in your particular plan, the card could decline it immediately.

Everyday household products

CMS distinguishes ordinary health-related over-the-counter products from general household purchases. Under a standard OTC benefit, potentially excluded items include:

  • Paper towels, tissues, and toilet paper
  • Cleaning and disinfecting products
  • Shampoo, conditioner, and hair dye
  • Deodorant and shaving supplies
  • Water bottles and personal coolers
  • Breath mints, gum, and teeth-whitening products

Some may qualify under a separate chronic-care benefit, but only when the plan covers them for that member.

Items your plan simply leaves off its list

There is no universal list of products that every Medicare Advantage flex card must cover. One plan might include sunscreen, vitamins, dental-care products, and compression supports, while another may cover a narrower selection.

The new rule requires plans to disclose eligible OTC items and identify which benefits members can access with a debit card. Check the plan's Evidence of Coverage or approved-product catalog rather than relying on a retailer's "flex card eligible" label.

Products sold at an approved store

Shopping at a participating pharmacy or grocery store does not mean everything in the building qualifies. The card's real-time system must confirm that the specific benefit is covered, not merely recognize the retailer.

A mixed basket could therefore include approved and unapproved products. Be ready to separate purchases or use another payment method for anything the card does not cover.

Groceries not included in your food benefit

Even when a card includes a food allowance, the plan controls which foods qualify. Do not assume every grocery item, beverage, prepared meal, or household essential is covered simply because the card worked at that store before.

Ask the plan for its current approved-food list. If its materials describe only broad categories, customer service should be able to provide more specific guidance.

Food benefits for members who do not meet every eligibility test

Some grocery and food allowances are Special Supplemental Benefits for the Chronically Ill, or SSBCI. Beginning in 2027, plans must verify that a member satisfies all three parts of the federal definition: a qualifying complex chronic condition, a high risk of hospitalization or another adverse outcome, and a need for intensive care coordination.

Having diabetes, heart disease, or another listed condition by itself may not establish eligibility.

Benefits supported only by self-attestation

CMS says checking a box and declaring yourself eligible is not enough. Plans must make an objective determination, potentially using a health risk assessment, claims history, or similar information.

They must also decide that the particular benefit could reasonably improve or maintain the member's health or function. As a result, some members who expected a grocery allowance based solely on a diagnosis may not receive access in 2027.

Purchases that exceed a shared allowance

A flex card may combine several benefits into one spending allowance. For example, money used for OTC products might reduce what remains available for another covered category.

That can make an advertised dollar amount look more generous than it is. Before enrolling, ask whether each benefit has its own allowance or draws from one shared balance, when the balance resets, and whether monthly or quarterly funds expire.

Purchases made after the plan year ends

CMS is also requiring flex card benefits to be limited to the applicable plan year. A plan may let members keep the same physical card, but unused benefit dollars generally cannot carry into the next plan year.

Members also should not expect a required midyear reminder about unused supplemental benefits. CMS rescinded that notice requirement, leaving members with more responsibility for checking balances and expiration dates.

Eligible purchases affected by a card problem

A decline does not automatically mean the item is excluded. The card could malfunction, the retailer's system might misidentify the product, or an eligible out-of-network purchase might require reimbursement.

Plans must provide customer support and an alternative reimbursement process in qualifying situations. Save the receipt, photograph the declined item and shelf label if helpful, and call the number on the card before assuming you must absorb the cost.

Bottom line

Medicare Advantage flex cards may become less forgiving in 2027 as real-time checks limit spending to the items and benefits each plan actually covers. Reviewing the fine print before enrolling is one of the smart moves for seniors who value an OTC or grocery allowance.

After coverage begins, consider making a small purchase early in January instead of waiting until you need a large order. That gives you time to confirm that the card is active, learn which products qualify, and resolve any eligibility or retailer problems before they disrupt your budget.
